Ejaculation is the release of semen from the penis during the male orgasm. It normally is a pleasurable experience without any discomfort. However, it can sometimes be painful, a problem which is not uncommon. The pain varies and may begin during sex steadily increasing in severity or occurs just during ejaculation. It could feel like pain or a burning sensation and varies depending on the cause. Painful ejaculation is also known as dysorgasmia or orgasmalgia. Here are the common causes of painful ejaculation.
Prostatitis and prostate disorders
Prostatitis is an inflammation of the prostate gland (located between the bladder and the penis). The severity of the symptoms varies but a common one is painful ejaculation. Prostatitis can be infectious or non-infectious. If it’s non-infectious it could go away on its own. Infectious prostatitis is treated with antibiotics. A condition called benign prostatic hyperplasia (BPH) can also cause the prostate to enlarge causing painful ejaculation.
Seminal vesicle problems
The seminal vesicle is the gland in which the sperm mixes with other fluids to make semen. One of the symptoms that something is wrong with the gland is the formation of hard growths called calculi which can cause painful ejaculation.
Cystitis
Cystitis is an infection of the bladder that is more common in women than men although it does affect men. Many of the men affected are those who have an enlarged prostate. The infection is treated with antibiotics. Left untreated, it can lead to complications like kidney or blood infection.
Stones or cysts
Cysts or stones may develop in the ejaculatory duct creating a blockage that causes painful ejaculation. They can also cause infertility. Treatment includes a procedure in which the stones or cysts are removed.
Pudendal neuropathy
Pudendal neuropathy is a condition that causes pain, discomfort, or numbness in the pelvic area. It can be caused by nerve damage, diabetes, or multiple sclerosis (MS). It can make it hard to use the bathroom, have sex or even sit down.
Medication and antidepressants
Some pharmaceutical drugs can cause painful ejaculation as a side effect. Other side-effects of antidepressants include erectile dysfunction and low libido.
Psychological issues
Relationship and emotional problems can manifest as physical problems causing painful ejaculation. Depression, stress, and anxiety can radically affect a man’s sex life.
Sexually Transmitted Infections (STIs)
Trichomoniasis is a sexually transmitted infection that can cause painful ejaculation and pain during urination. It can be treated with antibiotics. Other STIs that can cause painful ejaculation include chlamydia, gonorrhoea, and syphilis.

Surgery or radiotherapy
Surgery can have a number of side effects with some procedures including among their side effects painful ejaculation. Prostate surgery, pelvic radiation, and hernia repair are a few such procedures. Side effects of surgery can be treated but often, they reduce with time. Any procedure around the pelvic area carries with it a risk of damaging the organs which can make ejaculation painful.
